Home Health Licensure
Home Care Advantage Inc. has corporate
offices in Indiana Pennsylvania and is
licensed by the PA Department of Health
to provide care within the health and
safety standards established by rules
and regulations. We undergo periodic and
unannounced surveys to ensure we meet
delineated requirements for your safety.
Home Care Advantage is Medicare and Medicaid
certified by the Federal Health and Human
Services, and we undergo additional surveys
to ensure we remain in compliance with
federal home health regulations.
As a Medicare certified Home Health Agency,
we conduct comprehensive assessments of
all patients (initially and at regularly
scheduled intervals), and participate
in the federal program Outcome Assessment
Information Set (OASIS). Home Care Advantage
is required to conduct The Home Health
Agencies electronically submit OASIS health
information on Medicare patients to measure
patient outcomes for quality improvement.
Does Medicare Pay for Care in
My Home?
Yes, Medicare can pay for care in your
home, called Home Health Care. Home Healthcare
includes healthcare services at home for
an illness, such as heart disease, or
after an injury. Home health care is cheaper
than hospital care or a skilled nursing
facility (SNF), and is equally as good,
and the nurses come directly to your home.
If you receive Medicare through a health
plan, be sure to contact your insurance
company to get more information on your
Medicare Home Healthcare Benefits. Medicare
pays for Home Healthcare to get you better,
and become independent in the safety of
your home or residence. To receive Nursing,
or Therapy care, you must go through a
Home Health Agency. The Home Health Agency
is Licensed, Monitored, and complies with
quality standards for your ultimate safety.
How Can I get Home Health Care?
If you are in the hospital, nursing home,
or rehabilitation facility, you or family
may ask your Doctor for a nurse or a therapist
come to your home. When you are discharged
from the facility, Discharge Planners
will give you a list of home health agencies
and you have the right to choose an Agency
to care for your needs. You can even ask
your doctor or staff in the office about
Home Healthcare.
Four Steps to receiving Home Healthcare
- Patient must be home bound, due to
illness, or injury making it difficult
to leave home.
- A physician must order services, writes
a plan of care that a Home Health Agency
carries out.
- Care is provided as visits, Family
members care for the patient when the
Agency is not there.
- Care is provided by a Professional,
such as RNs, LPNs, Therapists and more.
